my survey on Satanism, titled 'Satanism and the Self.' It is completely
anonymous.
It is 10 quick, easy questions that takes an average of 5-15

Have you checked out the Satanism Surveys done by James R Lewis? Hes been compiling them for years and they are quite extensive. The last one he had was in 2011
